What are floaters?

Floaters, also called mouches volantes, are small, moving spots, strings or dots that appear in the visual field. They are caused by tiny protein fibers or cell debris in the eye's vitreous, which cast shadows on the retina. Floaters move with eye movements and are especially noticeable when looking at a bright background, such as a blue sky or a white screen.

They often occur with vitreous detachment, a natural aging process in which the vitreous detaches from the retina. In most cases, floaters are harmless, but a sudden increase may indicate a retinal problem and should be examined by an ophthalmologist.

Symptoms of floaters

Floaters, also called mouches volantes, appear as moving specks, strings or threads in the visual field. They are especially visible when looking at a light background, such as a blue sky or a white screen. Floaters can take several forms, including:

- Dark or gray dots, threads or cobweb-like structures

- Moving shadows that appear to float along with eye movements

- A sudden increase in floaters may be accompanied by flashes of light (photopsia), which may indicate vitreous detachment or a retinal problem

Although floaters are usually harmless, if there is a sudden increase or side effects such as flashes of light or a shadow in the visual field, it is important to consult an ophthalmologist immediately. This may indicate a retinal detachment, which requires emergency treatment.

Causes of floaters

Floaters occur due to vitreous detachment. In the eye, the vitreous is attached to the retina and fills the entire posterior chamber of the eye there. With age, the composition of the vitreous changes. It shrinks slightly and the jelly-like substance or vitreum becomes more fluid. As a result, the vitreum sac will suddenly detach from the retina, creating a space between the retina and the vitreous. This is called posterior vitreous detachment. This space will fill with water. During this process, impurities also form in the vitreous which will be perceived by the patient as black spots, "mouches volantes" or floaters.

These floaters can be very disturbing. They are especially noticeable in bright light, when looking at the blue sky or when looking at a white wall. The treatment for floaters is a vitrectomy.

Risks of vitreous detachment

A posterior vitreous detachment is usually harmless, but in some cases it can cause complications:

- Retinal tear or detachment: Strong traction on the retina can cause a tear, leading to a retinal detachment, an ophthalmic emergency.

- Severe floaters: Large or numerous floaters can interfere with vision and remain a nuisance.

- Vitreous hemorrhage: A tear in a blood vessel can cause a hemorrhage in the vitreous, causing blurred vision or dark spots.

- Flashes of light (photopsia): Traction on the retina can cause flashes of light, especially in the dark or with rapid eye movements.

A sudden increase in floaters, flashes of light or a black shadow in vision requires an emergency checkup with the ophthalmologist to rule out a retinal tear or detachment.

Treatment of Floaters: Vitrectomy

A vitrectomy is not always necessary. If the symptoms are limited and the patient does not consider the floaters disruptive, no treatment is necessary.

If the floaters are disruptive, they can be removed through a vitrectomy. Focus Eye Clinic is one of the only centers in Belgium where extra-mural vitrectomies are performed.

What is a vitrectomy

A vitrectomy is a surgical procedure performed to treat conditions in the vitreous and retina of the eye. The vitreous, a clear gel-like substance that fills the inside of the eye, is partially or completely removed during the procedure. This space is then filled with a special liquid, gas or silicone oil, depending on the type of treatment.

Vitrectomy is used for a variety of eye problems, including retinal detachment, macular surgery (such as an epiretinal membrane or macular hole), severe vitreous hemorrhage or infection, and complications of diabetes of the retina. Removing the vitreous allows the ophthalmologist better access to the retina to repair it or perform other treatments.

The procedure is usually performed under local anesthesia and takes an average of 20 to 60 minutes, depending on the complexity of the problem. Some recovery time may be required after surgery, depending on the filler used and the type of procedure. Vitrectomy is a safe and effective procedure that often leads to significantly improved vision or preservation of vision.
