When is lower eyelid surgery indicated?
Dermatochalasis in medical terms indicates excess skin thv the lower eyelids. The term derives from Greek: "Derma" refers to "skin. Chalasis' in turn means 'drooping'.
Dermatochalasis is often accompanied by excess fat in the lower eyelids which causes the fat pockets and circles around the eyes.
Unfortunately, there is little you can do to prevent the appearance of eye bags. There is a genetic factor that determines the extent to which eye bags are present. But age combined with gravity also play an important role.
After all, with age, your skin loses some of its elasticity. This causes it to sag and droop. The subcutaneous tissue also slackens, allowing fat to accumulate in the skin folds and create eye bags.
These three elements can occur separately or in combination:
Sagging or wrinkling of the skin
Sagging of the lower eyelid muscle
Sagging of the deeper membranes that hold up the fat of the lower eyelid
In addition to these phenomena, 2 additional aspects may occur:
Sagging of the outer corner of the eye
Deepening of the circles under the eyes.
Different techniques to remove lower eye bags

Blepharoplasty
Lower eyelid blepharoplasty involves spreading the excess fat in the eye bags into the underlying dark circles instead of simply removing it.
This kills two birds with one stone. The eye bags disappear as well as the dark circles that often appear under the eyes.
Despite the more extensive technical aspects of corrective surgery on the lower eyelids, this procedure can also be performed smoothly under local anesthesia, with or without light sedation.

Canthopexy - Canthoplasty
When the removal of excess skin and fat proves insufficient to eliminate eye bags, in many cases a suspension of the lower eyelid is also necessary.
This suspension technique is called a canthopexy and returns the corner of the eye to a higher position, as in young people, while simultaneously tightening the lower eyelid edge.
If there is severe sagging of the lower eyelid edge, then an additional shortening is needed: canthoplasty.

Progression of a lower eye elevator step by step

Step 1: Preoperative screening
A thorough pre-operative screening is essential for a successful blepharoplasty. During this consultation, the surgeon assesses the patient's general health, the specific anatomy of the eyelids and any medical conditions that may affect the procedure or recovery. Aspects such as excess skin, muscle strength, fat deposits and the position of the eyebrows are carefully analyzed to create an optimal treatment plan.
In addition, your expectations are discussed, and the surgeon explains the procedure, including possible risks and aftercare. If necessary, additional tests, such as a vision test or ophthalmic checkup, may be performed to ensure that the procedure is safe. Proper preparation helps not only to improve results, but also to ensure a smooth and comfortable course of treatment.

Step 2: The operation
Prescriptions:
Treatment with blood thinner should be stopped 7 days before surgery.
On the day of surgery, it is best not to drive yourself. So come with an escort! Taking public transportation is also not recommended.
Anesthesia:
The procedure is usually done under local anesthesia with or without a small sedative pill.
The procedure
A skin ellipse is removed according to a precisely defined pattern. The resulting scar is virtually invisible.
If the excess skin is accompanied by an internal sagging fat sac that curves on the inside of the lower eyelid, this fat sac is removed during the same procedure.

Step 3 : After the operation
After the procedure, we recommend observing the following:
Do not lie with your head down (e.g., when lacing your shoes) or completely flat. Sleep supine with a double pillow.
Avoid crowds and try to stay calm. Blood pressure should remain low.
Reading and watching television are fine.
Aftercare:
Post-treatment consists of anti-inflammatories and analgesics combined with cooling (no ice or coldpack directly on the eye, best wrapped in a thick piece of cloth, or clean washcloth moistened with cold water).
For 1 week, eye drops (artificial tears) and an antibiotic ointment are applied to the scar.
After 1 week, the sutures are removed.
